About the Role 

The role exists to provide support for the successful delivery of Short Term Restorative Care (STRC) and Home Care Packages (HCP) through clinical coordination and to provide specific assessments, interventions and planning, predominantly in Aged Care and NDIS communities, to increase peoples function and independence.

STRC is a Commonwealth funded program designed to provide early intervention which aims to reverse and/or slow “functional decline’ in older people and improve wellbeing through the delivery of a time-limited (up to 56 paid days), goal-orientated, multi-disciplinary and coordinated range of services designed for, and approved by, the client.  Annecto delivers STRC in a home care setting with focus on the Aboriginal and Torres Strait Islander Peoples, CALD and LGBTIQ+ communities. 

Requirements:

  • Recognised qualification in your Allied Health profession *Note: Professional registration to practice required and APRHA registration where applicable
  • Demonstrated experience in the community (3-5 years preferred)
  • Have a thorough understanding of complex needs associated with aging and disability
  • Demonstrated understanding of person centred practices
  • Experience in capacity building and improving functional independence
  • Previous experience in support facilitation/coordination in a community setting
  • Excellent communication skills, both written and verbal.
  • Developed skills in the use of computer software – Word, Excel, Internet and email.
  • A current valid Australian Drivers Licence and reliable, insured vehicle.

What would be great to have:

  • Experience as a practitioner in disability or aged services, or equivalent other experience
  • Experience working in a similar capacity in a community based not-for-profit organisation
  • Demonstrated ability to work effectively with a culturally diverse group.
  • Knowledge of local community resources/professional services which support individual
  • Demonstrated ability to work autonomously with a high degree of initiative as well as able to work cooperatively as a member of a professional team
