Case Manager – ACT and region
Due to significant growth we are looking for a Case Manager! This is an exciting opportunity to work with an independent, not for profit organisation located in Mawson working with a small, friendly and client focused team. As well as Case Management with our Home Care Package clients you will also be actively involved in service promotion, providing advice on access to community based aged care to community members and working closely with our team of wonderful direct Support team members. As well as Home Care Packages, we also provide Short Term Restorative Care, CHSP Other Food Services, brokerage to other agencies and private fee for service.
