About the Role 

This is exciting position will provide ongoing Nursing Assessment, monitoring, Review and support of individual care needs, as well as the provision of training to our team of Support Workers. This role support individuals receiving support through all programs delivered by Annecto in Mid North Coast Region.  Service model employed supports a Person Centred, Consumer Directed Care (CDC) approach, this approach is most suited to developing goal orientated individual plans supporting a care continuum.  Small case load STRC or HCP customers/Support Work required when necessary.

 

What we need from you: 

  • Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia
  • Experience in clinical care together with undertaking assessments and an understanding of Case Management
  • Demonstrated experience in working with people at all levels in a medium to large organisation with multiple sites
  • Experience in relevant health/social sciences.
  • Previous experience in a community setting.
  • Understanding and commitment to implementing Consumer Directed Care and Person Centred approaches.
  • Knowledge of local community resources/professional services which support individuals.
  • Demonstrated ability to work effectively with a culturally diverse group.
  • Demonstrated ability to work autonomously with a high degree of initiative.
  • Excellent communication skills, both written and verbal.
  • Developed skills in the use of computer software – Word, Excel, Internet and email.
  • A current valid NSW Drivers Licence and reliable, insured vehicle.
